What is the Self-Determination Program?
The Self-Determination Program (SDP) is a program designed to give individuals with developmental disabilities more control and flexibility over the services and supports they receive. The goal of SDP is to empower individuals to make choices about how they live their lives and how services are provided to meet their personal needs and goals.
Who is eligible for the Self-Determination Program?
To be eligible for the Self-Determination Program, you must:
Have a developmental disability, as defined by California law.
Be enrolled with a Regional Center.
Agree to follow certain rules, such as participating in an orientation and managing your budget responsibly.
Not be living in a long-term health care facility (unless you're in the process of transitioning out).
What is person-centered planning?
Person-centered planning is a guide using each individuals unique needs and goals to create a supportive plan. Each individual’s goals and what is needed to reach those goals, are outlined, including necessary services and supports. This highlights what is important to and what is important for the participant. Person centered planning can also include other family, friends, and anyone else the participant chooses to include.
The person-centered planning process ultimately leads to a written plan that is consistent with the person’s needs and desired outcomes, including the person’s goals and preferences in areas such as recreation, transportation, friendships, therapies, home, employment, and family relationships.
